There are thyroid receptors on every cell of the body, that is why it affects so many organ systems. The thyroid must be optimized to achieve better health and vitality. The pituitary gland releases TSH to talk to the thyroid. The thyroid organ then releases T4 (storage hormone) which much be convereted to T3 (active hormone) to be used at the cellular level. Genetic mutations have been found in individuals that decrease the conversion of T4 to T3. Stress, poor nutrition, heavy metal exposure, emotional stress can decreased this conversion. A patients TSH test may be normal but they are cellularly hypothyroid with symptoms. The T3 must be optimized a patient to have the cellular energy to have proper function and for symptoms to resolve.
The TSH test is inadequate to identify cellular thyroid function and T4 only replacement (Synthroid) is inadequate for many patients. A Free T3 and Reverse T3 tests are always ordered and often will add a Thyroid Peroxidase Antibody and Anti-Thryoglobulin Antibody tests to rule out Hashimoto's thyroiditis (the leading cause of hypothyroidism in the United States).
Natural desiccated thyroid replacement (T3/T4) and bioidentical compounded T3/T4 combinations are used. Sustained release options are also used. Ultrasounds of the thyroid are ordered if needed to assess nodules or inflammation.
